Terms & Conditions for NAFCM Program Members
Receiving NAFCM's Member-Rate on Case Manager
NAFCM program members are eligible to receive substantially discounted pricing on Mediate.com's Case Manager. To qualify for this pricing, your program must (1) be a current NAFCM program member, and (2) agree to the terms and conditions detailed below. By receiving a license for the Mediate.com Case Manager at NAFCM's special discounted pricing, you are confirming your agreement to the terms detailed below. If you have any questions about these terms prior to adopting the Case Manager system, please contact us at firstname.lastname@example.org. We encourage you to bookmark this site, as these terms may update without prior notice to applicable programs utilizing Case Manager.
NAFCM Data Conveyance Agreement
Conveyance of Anonymized Data to NAFCM. Contingent upon and in recognition of the discounted rates for the Mediate.com Case Manager product you or your entity received by benefit of your program membership in the National Association for Community Mediation (NAFCM), you hereby agree to allow Resourceful Internet Solutions Inc. to transmit to NAFCM certain anonymized data entered into and stored within your licensed copy of the Mediate.com Case Manager.
Data to be transmitted to NAFCM may include those data sets which do not compromise the confidentiality of your entity’s clients, the specific terms of their agreements, or other service details and/or outcomes reasonably understood to be private, protected, and/or confidential in nature. Specific data authorized for transmission includes, but may not be limited to, case types, referral sources, and outcome designations. At no time shall the personally identifiable information of your entity’s clients be shared with NAFCM.
Any modifications to this agreement must be made in a writing executed by both parties, by your online acceptance of updated terms, or after your continued use of the Mediate.com Case Manager at NAFCM program member pricing after such terms have been updated by NAFCM. This agreement shall remain valid throughout the duration of your licensed copy of the Mediate.com Case Manager, or until an authorized representative of your program requests in writing that no data from your program be transmitted to NAFCM. Opting out of this anonymized data conveyance agreement disqualifies your program for the special pricing offered to current NAFCM program members, though usage of the Case Manager system would remain available at Mediate.com's published pricing schedule.
The purpose of the conveyance of certain anonymized data to NAFCM is for the national statistical reporting and analysis of community mediation programs’ activities. Your agreement to this conveyance represents an express permission in writing by you as outlined in Section 11.4 (Protection of Your Data) of the Terms and Conditions within Resourceful Internet Solutions, Inc.’s EULA Agreement.
