Membership Categories and Online Applications
On this page, you will find applications for membership available in two formats. The first is an on-line form that you may complete and submit to NAFCM via email. We ask that you send membership dues payment (check or money order U.S. dollars only please) via snail mail as soon as possible. The dues amount is on the application. At this time we do not accept payment by credit card. NAFCM will also send you an invoice for dues payment. You may also open the membership application in Adobe® Acrobat® Reader in .pdf format. This allows you to print the form and complete it by hand. Please submit this form with dues payment.
